Administration of Medication

 

If a physician deems it medically necessary for a student to take medications during the school day, either prescription or non-prescription (these include, but are not limited to over-the-counter pain relievers, allergy medications, cough medications, TUMs, cough drops, etc), the Authorization for Medication form (found on this web site) must be completed by the parent and physician.  The form and medication (in the original pharmacy labeled container or if over-the-counter,  in the original bottle, box or bag) must be brought to the school by the parent/guardian.  A new form is required for each medication change, dose change and for each school year.
